The promise is simple. Use PROPECIA for 12 months. If you don’t at least maintain the hair you had when you began treatment, we’ll give you a full refund of your purchase price, less any rebates you received.

Program basics:

• This program is for new male patients aged 18 or older with male pattern hair loss who continue to use PROPECIA for 12 months.
• If you take PROPECIA for 12 months and you do not maintain your hair on the vertex (top of head) and anterior mid-scalp (middle front of head), you will be eligible for a full refund of your purchase price, less any rebates you received.
• Refunds will be given only if you complete 12 months of therapy (defined as the purchase of 360 tablets of PROPECIA) within a 12-month period.
• Your doctor’s signature is required to verify your results and qualify you for your refund.

Terms and Conditions:

Enrollment

• If your doctor feels PROPECIA is appropriate to treat your male pattern hair loss, get a prescription for PROPECIA.
• To enroll in the program, you must complete the enrollment form and mail it to Merck & Co., Inc. (Merck), at the address on the form.
• You must start treatment with PROPECIA between July 1, 2006, and December 31, 2008, to be eligible for this offer. Your enrollment form must be postmarked on or before January 15, 2009.

Reminders from Merck

• You must agree to receive mail, e-mail, and other information from Merck about hair loss and PROPECIA over the course of your 12 months of therapy.
• You may withdraw your consent to receive communications from Merck at any time, but if you withdraw your consent, you will no longer be enrolled in or be eligible to participate in the program.

Reimbursement Process

• If, after 12 months, your treating doctor determines that you have not maintained hair on the vertex or anterior mid-scalp, you or your doctor should contact Merck at 1-888-77-MERCK to request a reimbursement form.
• To receive the refund, both you and your treating doctor must sign the reimbursement form, and you must submit the form to Merck along with the original receipts from a licensed US pharmacy or a dispensing doctor’s office. Submissions must be made in the business reply envelope accompanying the reimbursement form.
• Receipts must total the price paid for 360 tablets of PROPECIA and must include the product, purchase price, quantity dispensed, dispensing date, and your name. The initial purchase must be made no more than 60 days before your enrollment date. The final purchase must be made no later than 12 months from your initial purchase. This offer is good only in the United States on PROPECIA dispensed from a licensed pharmacy or doctor in the United States who acquired the product directly or indirectly from Merck in the United States.
• This offer cannot be combined with any other offers for discounts unless authorized by Merck.
• Purchases from online pharmacies qualify for this program only if the pharmacy is VIPPS™ certified. The list of VIPPS™-certified pharmacies may be found at www.nabp.net/vipps/consumer/listall.asp.
• If you are eligible to receive reimbursement for or a price reduction of the cost of your prescription for PROPECIA under Medicaid, Medicare, or similar federal or state programs; private insurance; a health maintenance organization; a pharmacy benefits manager (PBM); or another third-party healthcare program, you are not eligible to participate in this program.
• You are eligible for only 1 refund under this program.
• Merck reserves the right to modify or discontinue the program at any time for any reason.
• All requests for reimbursement must be postmarked by June 30, 2008. Reimbursement forms submitted after June 30, 2008, are not eligible for a refund.
• This offer is void where prohibited by law.
• This offer is good for MEN ONLY.

Selected Important Information

• Women who are or may potentially be pregnant must not use PROPECIA and should not handle crushed or broken PROPECIA tablets because the active ingredient may cause abnormalities of a male baby’s sex organs. If a woman who is pregnant comes into contact with the active ingredient in PROPECIA, a doctor should be consulted. PROPECIA tablets are coated and will prevent contact with the active ingredient during normal handling, provided that the tablets are not broken or crushed.
• PROPECIA was developed to treat mild to moderate male pattern hair loss on the vertex (top of head) and anterior mid-scalp area (middle front of head) in MEN ONLY. There is not sufficient evidence that PROPECIA works for receding hairlines at the temples.
• PROPECIA is for the treatment of male pattern hair loss in MEN ONLY and should NOT be used by women or children.
• You should not use PROPECIA if you are allergic to finasteride or any of the ingredients of PROPECIA.
